  • £22,571 per annum (pro rata if part time)

  • 37 hours per week

A great opportunity to join G&G Worldwide Limited for work within the Adult and Social Care sector as an Independence and Wellbeing Practitioner, working from sites across Suffolk with hybrid working options including some working from home.

This is an exciting opportunity to join the locality teams in Suffolk to support people with assessment and review to ensure they have the right service at the right time with the right impact to achieve the best outcomes to maximise their independence.

Your role and responsibilities

You will work as part of an Integrated team and duties will include undertaking Care Act Needs Assessments, Carers Assessments and Social Care Reviews.

You will utilise your assessment skills, interpersonal skills and promoting the person's strengths and potential for independence, reducing the need for more intensive services including prevention of hospital admission.

The team

We are a passionate and supportive team, who are committed to the residents of Suffolk. We work in an integrated office with our health and housing colleagues. The Integrated Neighbourhood Team includes Adult and Social Care and Community Health Professionals who work closely together to ensure the best and most positive outcomes for the residents in our area.

There are lots of opportunities to develop your skills and experience and we have an apprenticeship team for those who wish to progress further to qualify as Social Workers or Occupational Therapists.

Travelling requirements for your role

Due to the nature of this role, you will require a driving licence and access to personal transport.

This role supports hybrid working, a broadly even balance between working from home and working from an office base. If you wish to discuss how that might impact your working arrangements.
